Have you ever bought cards and gotten something unexpected in your purchase? This has happened to me on more than one occasion, first when I bought the Machine Re-Volt deck, I got 41 cards, I got and extra Ancient Gear Gadjiltron Dragon.
This recent time I got a pack of Duelist Genesis cards and it came with 21 cards.

I once got about 5 different Duelist Genesis packs that only contained 4 cards, none of which were rare. In a couple Light of Destruction's I had two packs where only 8 cards in inside, missing the rare, and I have had a couple Crossroads of Chaos where I have gotten 10 cards, where one card is an exact duplicate of another card in the pack (common, unfortunately).

I also once got a pack in which the tops of the cards were cut off and they had excess border on the bottom.

Severed Restriction I also once got a pack in which the tops of the cards were cut off and they had excess border on the bottom. This happened with my Marshmallon from a Premium pack, only on the sides as opposed to top/bottom. I traded it for a United we Stand, though.

A guy here bought a Black Rose Dragon tin and in one pack he got 4 1st edition, ghost rare, Black Rose Dragons.

i gotten a couple of packs where my cards had scrape marks/creases
which sucks if it happens on a good pull....

Well this is more a misprint than a mishap but I pulled a ghost rare Chaos Neos and it had the name Rainbow Dragon xD

Got a CSOC pack with only 6 cards. The 6th was Secret Village.

I got a Dark Emperor deck where half the cards were missing most of the holographic tile in the lower corner. They'd only have a thin strip of two adjacent sides, and the rest was missing. (Still love the deck though)

2012 DJ Dragon Severed Restriction I also once got a pack in which the tops of the cards were cut off and they had excess border on the bottom. I've gotten a splendid venus like that, and i still have every intention to use it I mailed Konami about the cards and I got awesome compensation for it, a booster box of my choice :3
