Leprosy.
Dying while knowing that you could've done something to save the one you love, yet you didn't.
Having the flu and holding over the edge of the bed and falling, hitting your windpipe on the corner of a chair, crushing your windpipe and slowly suffocating you, not being able to call attention to yourself.

The last one actually happened to my sister's friend's five-year-old brother. ;~~;

she is ice skating she hears nosie its the sound of ice cracking beneath her feet but befor she could even think about any possible escape she slip into the water she try to fly away but her wing are soaked a mystical creature pulls her out and manages to get her breathing but the water was to cold she has no chances of staying alive in one last chance effort the mystical creature pulls out a magical bag of dust which he pours on her, the mystical creature take her to a warm log cabin but it to late she already has ammonia and minutes later dies in his arms.

I think it's....taking a dump in the toilet and then you found out that it was a large powerful vacuum and then you're intestines gets sucked up in that vacuum.
